HOW TO ENROLL IN SCHOOL

Children age 5 by November 30th are eligible for enrollment in the North Salem Central School District.

Kindergarten Registration 2009 School Year

Registration for kindergarten is scheduled for 3:30-6:30pm Monday, February 2, 2009 through Friday, February 6, 2009 at PQ Elementary School, 173 June Road in North Salem.

General Registration Information

Please be prepared with the following:

• Birth certificate or baptismal certificate

• Proof of residency
  Deed, mortgage document

• If renting, acceptable proof of residency would be a rental agreement with landlord’s residency information

• Health History Form
  All new students require a physical exam within 12 months prior to the date of enrollment

• Immunization Record

• Proof of guardianship
  If applicable
