• PHYSICAL EXAMS:

    The State of New York requires that physical health of each students’ development be monitored in grades Kindergarten, 1, 3, 5, 7, 9, 11, with the exception of sports physicals (please refer to MS/HS school nurse for more information regarding medical requirements for sports). This is a change from previous requirements. If you have any questions, please contact your building school nurse.

    The only form that may be used to document health examinations for students attending public school is the following:

    ·         The new NYS School Health form is not required for physicals done prior to 7/1/2018.

    ·         In 2019-2020 ONLY the approved form will be accepted.

    Dental Health Certificate: Is recommended for same years that health examinations are required.
