USE OF FACILITIES

Request & Permit form must be submitted 60 days prior to your event

  • Additional information may be requested from insurance company or vendor
  • Custodial fees may apply for outside organizations

Please follow the steps below when submitting your request:

STEP TWO

Insurance

Upon approval you will be required to provide the district with the appropriate insurance coverage.

INSURANCE CERTIFICATES NEED TO BE RENEWED ANNUALLY. Upon receipt of the certificate provided to you by your insurance carrier please review the effective dates to avoid a lapse in coverage for the District.

  • Please review the insurance requirements below and send BOTH the Insurance requirements for facilities use and the sample required certificate of insurance to your carrier for reference.

STEP THREE

Workers Compensation & Disability

IF YOU ARE EXEMPT FROM WORKERS COMPENSATION and DISABILITY PLEASE SUBMIT FORM CE-200 (Please click the link below and print and sign the document for submission)

Workers Compensation and Disability Certification of Attestation of Exemption (CE-200) 

STEP FOURFood Trucks 

In addition to the facilities use forms and the insurance requirements you will need a Temporary Food Service permit issued by New York State (click following link):  NYS Food Service Vendor Requirements

The following supplemental documentation must accompany the Certificates of Insurance for Food Trucks

LIST OF APPROVED VENDORS FOR THE 2025-2026 SCHOOL YEAR 

FOR YOUR SAFETY AND ENJOYMENT PLEASE ADHERE TO THE FOLLOWING RULES FOR USING THE TRACK OR FIELDS

  • Track and Fields are closed to the public during scheduled Physical Education activities or Athletic Events
  • Water beverages only on the track or fields
  • Sneakers or athletic shoes only
  • No animals or pets of any kind
  • No food or alcoholic beverages
  • No chewing gum
  • No glass containers
  • No smoking, vaping, etc.
  • No strollers of any kind are allowed on the Track or Field 
  • No bikes, scooters, skateboards or rollerblades, etc.
  • No metal cleats
  • No golf playing
