NEWS FROM THE NURSE
The New York State Education Department has changed the Health Appraisal Form required for all physicals. Effective 7/1/2021 only the specified form (Revised in 2020) is acceptable. All other Health Appraisal Forms will be considered invalid.
During these uncertain times, it may be difficult to decide whether or not your child should attend school if they are not feeling well. Feel free to reach out for guidance with any medical questions or concerns regarding your child's symptoms or illness. Also, please do not send your child to school if they're not feeling well. I am happy to help you through the process of making sure your child should attend school.
